Here's what you can do to help prepare the individual you referred for a successful first appointment:

Share Forms

We need each patient to complete a Consent Form and Release of Information before their first appointment. Please share the links to these forms with the individual you referred.

Check Notifications

If you have opted in for notifications, you will receive Patient Status Updates to let you know when the individual you referred has scheduled their first appointment.
