MINIMUM REQUIREMENTS: **FMLA, Short-term Disability and Long-term Disability: Must have active engagement in medication management services for a minimum of 3 months and have completed 4+ medication management visits. There are fees associated with completing this payable prior to forms completion. NOTE: Records take anywhere from 10-14 business days to process after consent is signed. Some records have fees associated with them.
