Brave Health is able to provide the following records: 


  • Letters of Services (including diagnosis, treatment dates, and medications as applicable)
  • Initial Biopsychosocial Assessment 
  • Psychiatric Evaluation
  • Psychiatric & Therapy Progress Notes
  • Treatment Plans
  • Safety Plans 
  • Discharge Summaries
  • Monthly Progress Reports 
  • Records for SSA
  • DCF Form 5339
  • FMLA, once minimum is met**
  • Short-term Disability (STD), once minimum is met**
  • Long-term Disability (LTD), one minimum is met**

Brave Health is not able to provide the following records: 


  • Emotional Support Animal (ESA) letters
  • Housing Documentation (ig. Section 8, reasonable accommodations) 
  • Clearance for Surgeries (ig. bariatric) 
  • Clearance for medications Brave does not provide
  • Clearance for military
  • Psychological Testing 
  • DUI/DWI feedback reports
  • Citizenship or immigration paperwork
  • Reasonable accommodations for schools or work (ig. extended lunch times, extra days off)
  • Withdrawal from school forms
  • Parental Fitness Evaluations
  • Letters for jury duty
  • Court appearances (unless issued a subpoena)
  • Student Loan Forgiveness
  • Excusal Letters / Return to Work letters

    **FMLA, Short-term Disability and Long-term Disability: Must have active engagement in medication management services for a minimum of 3 months and have completed 4+ medication management visits. There are fees associated with completing this payable prior to form completion. 

    NOTE: Records take anywhere from 10-14 business days to process after consent is signed. Some records have fees associated with them.
