Completing Your Patient Forms

Please check that your information is correct. This helps your forms link to your file right away.

• Use Your Full Legal Name: Use your legal first and last names. Do not use a nickname.
• Skip Middle Names: Only type your first and last name in the boxes.
• Double-Check Contact Info: If you have requested or completed an appointment with us before, use the same phone number and email. Make sure there are no typos.
• Use the Right Date Format: Check your birth date carefully before you submit the form.

1. Try a Different Web Browser

3. Fix Problems with Text / SMS Links
